MDH Medical Associates – LPN/CMA

Hours of Work: Monday through Friday 8AM – 4:30PM  (Full-time)

Qualifications: Graduation from accredited licensed practical nurse or post-secondary medical assistant training program, CPR certification, medical office experience preferred.  Previous EMR experience preferred.  Must be willing to meet the staffing needs in three clinical locations.  Highly motivated with excellent written and verbal communication skills.  Must be willing to work as part of a team to ensure excellent patient care.  Computer experience required.

Responsibilities: Assists with the nursing process in person and over the telephone.  Practices within scope and current standards of care.  Accountable for achieving quality standards of state, federal and regulatory bodies. Performs a wide variety of patient care activities for the clinic’s patients of all ages, including general nursing care, administration of prescribed medications and treatments, education of patients and families, and response to patient telephone calls, utilizing the electronic medical record (EMR) and ePrescribe medication ordering systems.  All care is provided under the supervision of the physician.


Switchboard/Patient Access Clerk

Hours of Work: Monday-Friday, 9:00AM – 5:30PM  (Full-time, 40 hours per week, Every 4th Saturday & rotating holidays)

Responsibilities and Qualifications

Must have high school diploma and good customer service skills.  Must be willing to learn CPSI computer system for registrations. Patient Access Representative is responsible for registration and financially related data collection when a patient arrives at the practice site. Cover switchboard and Main Desk when needed. Good customer service skills a must.


Health Information Management/Patient Access Supervisor

Hours of Work: Monday-Friday, 8:00AM – 4:30PM  (Full-time, 40 hours per week)

Responsibilities and Qualifications

Preferred 2 years’ experience in Health Information Management. Must have good communication skills, ability and confidence to interact with physicians, patient/family, nursing and other ancillary staff. Responsible for accurate efficient coding and abstracting within department. Responsible for enforcing policies and procedures of both Health Information Management and Patient Access. Must have an Associates in Health Information Management or Health Administration.


Environmental Services Tech

Hours of Work: Full-time, 10AM-6:30PM (some weekends required)

Job Summary

E.V.S. staff are responsible for the cleanliness and disinfecting process of the Hospital, clinics, outbuildings. They will achieve this by following strict guidelines for disinfection and sanitizing processes and follow set cleaning procedures.

Responsibilities and Duties

The core responsibilities include:

  • Cleaning and disinfecting Hospital and Clinic surfaces and equipment
  • Removing trash and bio-hazard waste.
  • Physically checking and locking doors.
  • Keeping E.V.S. equipment clean and organized.
  • Following all E.V.S. and MDH policies and procedures.
  • Monitor for any wear and tear and submit a work request to maintenance.
  • Specialty cleaning of lights, vents, ceilings. Etc.
  • Maintaining a positive and professional attitude at all times.
  • Mixing chemicals – following manufacturers guidelines.
  • Any additional tasks requested by the Director or lead E.V.S. person.
  • Documentation of all tasks.
  • On Call required.

Qualifications and Skills

  • Read/Write/Speak in English.
  • Current Illinois Driver’s License.

  • Must be 18 years of age or older.

Havana Medical Associates/Specialty Clinic RN/LPN

Hours of Work: Monday – Friday, 8am – 4:30pm (Full-time/40 hours per week)

Qualifications:

Medical office and computer experience required. Electronic medical records experience preferred. Exceptional patient relations experience required. Must have the ability to be a self-starter, multi-task, prioritize and work as part of a team. Current, unrestricted Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license and BLS required.


Clinic RN

Hours of Work: Full-time (40 hours per week)

Qualifications:

Graduation from accredited registered nurse (RN) program, active, unrestricted RN license, CPR certification, medical office experience preferred.  Previous EMR experience preferred. CNA/Telemetry Tech

Hours of Work: 7:00pm to 7:30am, every other weekend  (Part-time)

Qualifications:

  • Telemetry experience or willing to learn telemetry.
  • Certified Nursing Assistant who has successfully completed a certification course and is listed on the State of Illinois CNA Registry.
  • CPR certification required.

Responsibilities:

The successful candidate will perform patient care as defined by scope of practice and collaborate with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes under the direct supervision of a Registered Nurse.


MAINTENANCE MECHANIC

Hours of Work: Full-time, 7:00AM-3:30PM (some weekends required)

Job Summary

Maintenance Personnel are responsible for general facility maintenance including Preventive Maintenance, Work Orders, Plumbing, Carpentry, Minor Electrical, General Groundskeeping and Landscaping. Jobs are in various locations across all Mason District Hospital facilities. This position reports to the Director of Plant Operations.

Responsibilities and Duties

The core responsibilities include:

  • Preventive Maintenance on most hospital equipment.
  • Work Order requests for various jobs submitted by different departments.
  • Snow Removal
  • Be familiar with all departments policies and procedures
  • Preventive Maintenance, Work Orders and Plumbing.
  • Carpentry, Minor Electrical, General Groundskeeping and Landscaping
  • Weekend and on-call hours are required.

Qualifications and Skills

  • High School Diploma or GED.
  • Read/Write/Speak in English.

Surgical Scrub/Central Sterilizing Tech

Hours of Work: Monday-Friday (flexible to meet surgery schedule) (Full-time)

Responsibilities: A highly talented Surgical Technologist with experience assisting in operations, under the supervision of surgeons, registered nurses, or other surgical personnel; helping to set up the operating room, preparing and transporting patients for surgery, adjusting lights and equipment, passing instruments and other supplies to surgeons and surgeon’s assistants, holding retractors, cutting sutures, and helping to count sponges, needles, supplies, and instruments.

Qualifications:

  • High school graduate or equivalent
  • Scrub technician training program and certified in Illinois or at least 1 year on-the-job experience performing scrub tech duties

  • Licensed as a Speech Therapist by the State of Illinois.
  • Applicants must be a graduate from an accredited ST program with a minimum of BS in ST and must possess an Illinois license or be eligible.
  • Applicants must be highly motivated, experienced, professional with the ability to start programs, be a mentor, and work independently in the inpatient, outpatient, and home health settings.
  • One must have a current and valid Illinois Driver’s License, reliable transportation, and car insurance is required.

Occupational Therapist

Hours of Work: Full-time

Qualifications:

  • Licensed as an Occupational Therapist by the State of Illinois.
  • Applicants must be a graduate from an accredited O.T. program with a minimum of BS in O.T. and must possess an Illinois license or be eligible.
  • Applicants must be highly motivated, experienced, professional with the ability to start programs, be a mentor, and work independently in the inpatient, outpatient, and home health settings.

PHYSICAL THERAPIST

Hours of Work: 8:00 A.M. TO 4:30 P.M. Monday through Friday and weekends as needed on a rotation basis. (Full-time)

QUALIFICATIONS/RESPONSIBILITIES:   

Licensed as a Physical Therapist by the State of Illinois.  Applicants must be a graduate from an accredited P.T. program with a minimum of a Doctorate Degree in P.T. and must possess an Illinois license or be eligible. The Physical Therapist is responsible for providing highly skilled evaluations and patient care for in-patients, out-patients, swing-bed, early intervention (optional) and home health clients.  Physical Therapy clients may include infants, toddlers, preschoolers, adolescents, adults, and geriatric population.  Applicants must be highly motivated, experienced, and professional with the ability to start programs, be a mentor, and work independently in the inpatient, outpatient, early intervention and home health settings.  One must have a current and valid Illinois Driver’s License, reliable transportation, and car insurance is required.   One must possess good handwriting, excellent communication skills, good computer operation skills, and operation of patient care equipment.


Med/Surg Department- Registered Nurse

Hours of Work:  Night Shift, 6:30pm-7:00am (Part-time)

JOB SUMMARY: A Registered Nurse assumes responsibility and accountability for a set of assigned patients for a designated period of time.  During the designated time the RN will incorporate caring for all patients in all settings, and includes, but is not limited to, all of the following: A comprehensive nursing assessment every shift and as needed, the development of a plan of nursing care to be integrated within the patient-centered health care plan that establishes nursing diagnoses, and setting goals to meet identified health care needs, determining nursing interventions, and implementation of nursing care through the execution of nursing strategies and regimens ordered or prescribed by authorized healthcare professionals. The Registered Nurse will administer medication in a timely manner.  The nurse will delegate nursing interventions to comply with the patient’s current plan of care and advocate for assigned patients.  The Registered Nurse will evaluate responses to interventions and the effectiveness of the plan of care. The nurse must communicate and collaborate with other health care professionals as needed to ensure continuity of care. The nurse will be expected to participate in development of policies and procedures, and systems to support patient safety.

Qualifications: 

  • Graduated from an accredited Professional School of Nursing
  • Current Illinois License.
  • Willing to obtain ACLS, PALS, and/or TNS after 1 year of nursing unless PRN
  • Must be willing to cross train to other departments within MDH
  • Compassionate, understanding self-motivated and enjoys working with others.

  • Desire to expand current medical knowledge.
  • CPR certification required

Senior Advantage–Licensed Clinical Social Worker—Outpatient Therapist

Hours:  Monday- Friday, 8:00am-4:30pm (Full-time)

Requirements:

  • LCSW is required, Master’s degree in Mental Health, Counseling, Social Work or Psychology

Job Duties: The successful candidate will be responsible for treating ill and injured patients of all age groups. The EMT or Paramedic will perform skilled work while operating in an approved mobile medical unit, rescuing victims and performing advanced emergency care including the administration of life sustaining treatment to patients on-site and while in route to hospital. The candidate will be an active team member in the Emergency Department and be a positive influence in the organization.


Registered Nurse- Emergency Department

Qualifications:
